Ganciclovir for invasive cytomegalovirus infection in renal allograft recipients.

چکیده
CYTOMEGALOVIRUS (CMV) is a major infectious complication of renal transplantation associated with significant morbidity and mortality. 1-3 Ganciclovir inhibits herpesvirus DNA polymerase4 and has shown antiviraJ effects in vivo in several reported clinical studies. 5-8 We report our experience with the use of ganciclovir in treating tissue-invasive CMV infection following cadaver renal transplantation. The results suggest that ganciclovir is well tolerated and effective in arresting the progress of invasive CMV to life-threatening disease with little drug-related morbidity.

Incidence of ganciclovir resistance in CMV-positive renal transplant recipients and its association with UL97 gene mutations
Human cytomegalovirus (CMV) remains the most common infection affecting organ transplant recipients. Despite advances in the prophylaxis and acute treatment of CMV, it remains an important pathogen affecting the short- and long-term clinical outcome of solid organ transplant recipient. The emergence of CMV resistance in a patient reduces the clinical efficacy of antiviral therapy, complicates t...

Between November 1987 and September 1989, 419 cadaveric renal transplants were performed at our university. Of the patients 36 (8.6%) had invasive cytomegalovirus infection documented by gastric or duodenal mucosal biopsy in 23 (64%), bronchoalveolar lavage in 12 (33%), allograft biopsy or nephrectomy specimen in 5 (14%) and/or liver biopsy in 1 (3%). Cytomegalovirus severity was defined as mil...
متن کاملAntiviral agents for preventing cytomegalovirus infection in pediatric renal transplant recipients: A systematic review
Background and Objective: Cytomegalovirus (CMV) infections are associated with severe morbidity and mortality in patients, especially pediatric renal transplantation patients. The use of immunosuppressive agents places these patients at the risk of viral infections. As cytomegalovirus infection influences the graft outcome, adopting useful strategies for limiting this virus after transplantatio...
متن کاملCytomegalovirus infection in renal transplant recipients.
We have retrospectively analyzed the incidence of cytomegalovirus (CMV) infection in 250 consecutive renal allograft transplants performed in 244 recipients. The mean follow-up was 35.1+/-25.4 months. Immunosuppression was cyclosporine- or tacrolimus-based triple therapy. CMV infection prophylaxis with ganciclovir for 3 months post transplant was prescribed in CMV-seronegative recipients of all...
